Custom Form Field Types

These system fields help FSRevs associate a response with the form’s customer. Each can appear once per form.

Collects the customer’s name for identity and display.

Validates the customer’s email address and can show an optional email marketing-permission choice.

Collects the customer’s US or Canadian phone number and can show an optional SMS marketing-permission choice. Validation does not prove that the number can receive SMS.

Use a regular Text Input when an email address or phone number belongs to a spouse, employee, applicant, emergency contact, or anybody other than the form’s customer.

A single-line answer. Choose the appropriate mode for ordinary text, email, phone, URL, or number data. Available settings vary by mode and can include placeholder, length, and numeric limits.

A longer, multi-line text answer with optional placeholder and length controls.

A list of configured choices. It can allow one choice or multiple choices.

Displays several choices and allows multiple selections.

Displays several choices and allows one selection.

Collects an on/off response.

Collects a formatted long-form response. The respondent receives formatting tools, but this input does not provide managed Customer destinations or image blocks.

This is different from authored WYSIWYG Content. See where the Rich Editor is used for the capability differences.

Collects one value along a configured numeric scale.

Collects files from the respondent. A form can contain up to two File Upload elements, and each can accept up to six files.

Supported uploads include:

  • Images: JPG, PNG, WebP, GIF, HEIC, and HEIF
  • Documents: PDF, TXT, and Markdown

Each file can be up to 10 MB. Image uploads can also request marketing consent when the form should ask for permission to reuse submitted media.

Collects a date, time, or combined date and time. Depending on its mode, it can allow a single value, a date range, or multiple dates.

Display-only elements organize or explain the form but do not create answer values.

Adds visual separation between groups of elements.

Adds a structured heading.

Displays short instructions or supporting information in a styled callout. Notes intentionally provide fewer authoring tools than WYSIWYG Content.

Embeds supported YouTube or Vimeo content.

Adds formatted explanatory content without collecting an answer. It uses the Rich Editor and supports Customer destinations and image blocks.

Depending on the element, settings can include:

  • Label and helper text
  • Required state
  • Placeholder or default value
  • Choices and selection rules
  • Format or validation limits
  • Response key
  • Hide Element

Hide Element is a static setting. It does not create conditional rules based on another answer.

Response keys identify answers in stored responses, reports, and exports. Keep them stable after the form begins collecting data.

Customer Email and Customer Phone can identify the customer and request permission. Email and phone Text Input modes only validate response data. They do not update customer identity or grant marketing permission.
