Custom Form Settings Guide

Go to Flows and Forms > Form Builders, open a Custom Form, and select Build & Configure. If you can view the form but cannot edit it, FSRevs shows a read-only configuration summary and a safe preview instead of editable controls.

Settings are organized into four tabs:

  1. Basics
  2. Build
  3. After Submission
  4. Send & Share

Form Name identifies the form in the portal. Choose either a specific location or All Locations when creating it.

Location scope cannot be changed after creation. This protects existing requests, responses, and attribution. Create another form if you need a different scope.

For an All Locations form, enable Ask visitors to choose a Location when public visitors should identify the relevant physical location before the form opens.

Turn Accept New Responses off when the form should no longer accept new starts. This makes its public Smart Link and new request actions unavailable. A customer using an existing personalized request can still finish the response already issued to them.

The Journey Summary previews the main Form stages created by the current settings. It is not an observed Customer Journey from a submitted response.

Use Questions & Content to add, clone, reorder, collapse, and configure form elements. The form’s location scope must be resolved before building its contents.

The special Customer Name, Customer Email, and Customer Phone elements can each appear at most once. Other question and display elements can be used as needed, subject to their individual limits.

Use WYSIWYG Content when you need authored formatting, managed Customer destinations, or image blocks. Use the Rich Text Editor question when the respondent should submit a formatted answer. See where the Rich Editor is used for the capability differences.

Form Outline is a structural summary of the current element order. It is not an interactive copy of the customer form.

Use Preview Current Settings to see an interactive preview with unsaved changes. The preview stages are:

  • Location when the form asks a public visitor to choose one.
  • Form for questions and content.
  • Completion for the configured next step.

The preview uses example information and disables actions that would submit data, grant permission, or leave FSRevs.

Removing a question does not remove its answer from historical submissions. Changing a response key can make new and old answers appear as separate questions in reports or exports. Keep response keys stable once a form has collected data.

If FSRevs encounters an older saved element that the current builder cannot edit, it preserves that element during unrelated changes and displays a warning.

Choose what happens after a successful submission:

  • Simple Page — Open a branded FSRevs page.
  • Smart Link — Continue through a reusable Smart Link.
  • My Link — Open one of your managed website, review, or social links.
  • No next step — Show the form’s simple completion message.

If a previously saved target becomes unavailable, FSRevs preserves it while you make unrelated edits. Clear or replace it deliberately when you are ready to change the customer experience.

The Simple completion message appears when a public Smart Link submission has no next step. Embedded forms and previously issued requests may use the completion experience associated with that delivery method.

Configure the reusable delivery settings:

  • Email message template
  • SMS message template
  • Employee-name display format
  • Optional primary Smart Link
  • Link Preview

The primary Smart Link gives the form a reusable public entry point. Open the Smart Link record to add destination rotation, attribution configurations, and QR designs. See Smart Links.

An existing form can include:

  • Reports — Performance and answer summaries.
  • Activity — Responses locked to this form, with location context and export/send actions.
  • Build & Configure — Editable settings or a read-only summary.
  • Team Notifications — Notification rules when your access includes them.

The pages shown and the records available on them depend on your access.
